Risk Management and Insurance promotes the protection and safety of the University’s employees, students and visitors by: taking cost efficient steps to minimize the risk of financial impact as a result of loss to or destruction of assets; insuring, abating, or eliminating identified risks; ensuring that claims have been adjusted in accordance with contractual agreements; and establishing and implementing a standard method for setting real and personal property values.

Driver Authorization Request

Driver authorization is required for employees and students who drive on behalf of the University. The driver authorization requirement applies to the following:

  • Driving any University vehicle including golf carts and utility vehicles
  • Driving any rented, loaned, or leased vehicle on University business.
  • Driving other University employees or students in a personal vehicle while on University business.

Samford partners with Choice Screening to run motor vehicle reports. Complete this online consent form to initiate the driver authorization process. In most cases, results will be returned within 3-5 business days.

Business Travel Insurance

When traveling, you can now feel confident that you are in safe hands if an emergency arises. UnitedHealthcare Global provides medical, security and travel-related assistance services.

Always carry your identification card with you. Listed on the back of the card are the telephone numbers for the worldwide UnitedHealthcare Global network. When you call, be prepared to provide your id number, organization's name, your name and a description of the situation

Air Medical Transportation for Employees Enrolled in the University Health Plan

If a member is hospitalized while traveling more than 150 miles from home (calculated as a straight-line distance, not road miles) air ambulance transportation is available to transport the member to a network hospital of their choice near their home. Ground ambulance transportation is provided from the hospital to the aircraft and then from the aircraft to the receiving hospital. Air ambulance transportation is also available in some cases when a member needs specialized hospital services in a hospital located more than 150 miles from their primary residence so long as the hospital is located within the country of residence (United States or Canada only), the member is unable to travel by commercial means without a medical escort, and the transport is approved by us. This includes transport of transplant recipients.

There are no deductibles, copayments, or coinsurance applicable and there are no claim forms to file for this service. Members call a toll free hotline 1-877-872-8624 (available 24 hours a day, 7 days a week) to request air transport services. There are no restrictions on the number of travel days within the United States but services are available only twice per calendar year per member and are not available to members travelling outside the United States for more than 90 consecutive days. Services are also not available for (1) any location where the U.S. State Department has issued travel restrictions or declared to be high risk areas; (2) any member with tuberculosis or other chronic airborne pathogens; (3) in most instances, a member beyond the second trimester of pregnancy; (4) members with simple injuries or mild illnesses which do not require hospitalization.

Blue Cross Blue Shield Global® Core for Employees Enrolled in the University Health Plan

If you are outside the United States, the Commonwealth of Puerto Rico, and the U.S. Virgin Islands (hereinafter “BlueCard service area”), you may be able to take advantage of the Blue Cross Blue Shield Global® Core service when accessing covered healthcare services. Blue Cross Blue Shield Global® Core is not served by a Host Blue.

If you need medical assistance services (including locating a doctor or hospital) outside the BlueCard service area, you should call the Blue Cross Blue Shield Global® Core service center at 1-800-810-BLUE (2583) or call collect at 1-804-673-1177, 24 hours a day, seven days a week. An assistance coordinator, working with a medical professional, can arrange a physician appointment or hospitalization, if necessary.  

Submitting a Blue Cross Blue Shield Global® Core Claim

When you pay for covered healthcare services outside the BlueCard service area, you must submit a claim to obtain reimbursement. For institutional and professional claims, you should complete a Blue Cross Blue Shield Global® Core claim form and send the claim form with the provider's itemized bill(s) to the service center (the address is on the form) to initiate claims processing. Following the instructions on the claim form will help ensure timely processing of your claim. The claim form is available from us, the service center or online at www.bcbsglobalcore.com. If you need assistance with your claim submission, you should call the service center at 1-800-810-BLUE (2583) or call collect at 1-804-673-1177, 24 hours a day, seven days a week.
