The Institutional Review Board procedures have been steamlined for your ease of use. Here's all you need:
- IRB Research Tutorial - View this PowerPoint presentation to understand how to submit an IRB application.
- IRB Application Form - Fill out this form and submit it as an e-mail attachment to the appropriate school or department representative, listed below. You should receive a response about 30 days from the time you submit your application.

Human Subject Project Closure Form
Your research may also be subject to review by the Samford Survey Review Committee (SRC) if it involves surveys conducted with Samford students, employees or alumni. Determine if you must also seek approval from the SRC.

Overview & Membership
The Institutional Review Board of Human Subjects is one of four special committees appointed by the provost for a three-year staggered term. Membership consists of one person from the community with expertise in medical and/or scientific ethical issues and representation from each school which conducts research involving human subjects, specifically:
