Informed Consent Form Examples

Investigators are reminded that all EXPEDITED and FULL IRB applications require a subject informed consent form. EXEMPT IRB applications do NOT require a subject informed consent form.

Below are links to some subject informed consent EXAMPLES. Please note that your subject informed consent form may not be completely identical to the examples. Please use the examples to guide in preparing the subject informed consent form. Please read the section on the IRB webpage regarding subject informed consent (samford.edu/IRB/consent.html) content and purposes BEFORE preparing the subject informed consent form.

The wording of the following sections of the subject informed consent form should be identical to the examples:

  • Agreement
  • Questions (please include specific names and contact information)
  • Payment for Research Related Injuries
  • Withdrawal without Prejudice

The wording of the other subject informed consent form sections will vary from the examples, depending upon the type of research and level of risk.
