In order for the CTLS to provide optimum programs and services, it is important to obtain feedback. Thank you for attending a Center for Teaching, Learning and Scholarschip (CTLS) program.  By completing the evaluation form, you will assist us in identifying what areas were addressed well and what areas needed improvement.  All evaluations are strictly confidential and will only be used by the CTLS staff to determine the effectiveness of CTLS programming and to determine future educational needs. Upon completion of the form, you may fax it to (205) 726-4110 or mail it via campus mail to CTLS, Davis Library.
