In order for the CTLS to provide optimum programs and services, it is important to obtain feedback. Although, the specific quality improvement process is still being developed, you may contribute your thoughts and ideas on workshops using the Program Evaluation Form (pdf) or contact Mary Sue Baldwin, CTLS director, via extension 4097 or e-mail at msbaldwi@samford.edu. We appreciate your ideas!

Thank you for attending a Center for Teaching, Learning and Scholarschip (CTLS) program. By completing the evaluation form, you will assist us in identifying what areas were addressed well and what areas needed improvement. All evaluations are strictly confidential and will only be used by the CTLS staff to determine the effectiveness of CTLS programming and to determine future educational needs.
