How do I obtain a "Hard Copy" (Paper) of the Key Request form?

A paper copy of the Key Request form may be obtained from the bookstore,  from the main campus safety and security desk, and from the key shop.

Who is responsible for repairing doors and/or locking hardware?

Facilities Services is responsible for the repairing of all doors and lockset hardware. This means that if you are having a problem, or need assistance with anything besides the cylinder in which your key is inserted into (or the key itself) you should probably call Facilities Services at 726-2711 or visit the Facilities Services work order web page at:

When will the departmental charges (if any) be incurred by the department responsible for payment of charges?

In most cases departmental charges are submitted to the Controller's Office on the last business day of each month (assuming the work was completed).  Your department will normally incur departmental charges within the first two weeks of the month following the month in which the work (Key Request) was completed.
