Payment

There is a $15.00 fee for visits with the physician assistant.  Charges will also be incurred for lab services, medications, injections and medical supplies utilized during the visit.  There is no charge to see a nurse.  We do not accept any medical insurance in the clinic. All charges are billed to the student's account and are the responsibility of the student. Upon request, we will provide you with the necessary diagnosis (ICD-9) and procedure (CPT) codes to submit a claim to your insurance provider.

Attention: Blue Cross and Blue Shield subscribers should contact the office at 726-2835 when submitting claims to obtain the appropriate BCBS provider number.
