Consent to Release Confidential Student Information (aka the Orange Form)
The Consent to Release Confidential Student Information Form is for students who may have missed the opportunity to complete the form during an Orientation session, or for those who no longer want the confidentiality block in place.
In accordance with the Family Educational Rights and Privacy Act (FERPA), a college or university may not release confidential student information, including grades, to parents or guardians without (1) the written consent of the student or (2) written evidence from the parent that the student is a dependent of the parent as defined in Section 152 of the Internal Revenue Code. Either the student's written consent or the parent's adequate documentation of dependent status is required. A copy of the latest federal income tax form of the parent(s) or guardian can be used to document a student's dependent status.
If you give consent to Samford University to release to your parent(s) or guardian confidential student information relating to you, please sign the form linked below and return this document to the Office of the Dean of Academic Services (or Student Records).
If this consent to release information form is not executed and returned to Samford University, student information will not be released to your parent(s) or guardian unless adequate documentation of dependent status is provided to Samford University by the parent(s) or guardian. Upon receipt of adequate documentation of dependent status, Samford University may release confidential student information to your parent(s) or guardian even if you have not signed the consent form.
